This form may be used to do one of the following:
       1) request a book that is not listed in the library catalog
    2) request a non-print item without a next to the
title
  3) request the purchase of a new item for the New
Milford Library collection

Please DO NOT use this form to request an item that is already listed
in the New Milford or Bibliomation Catalog. You may request
these items directly from your catalog search by clicking
on the rectangular button that says Request Item.
