Homebound Patron Card Registration Form

Mansfield Public Library offers delivery of library materials to homebound patrons. Please read the following and fill out the form to start home delivery.

1. Deliveries are made by staff and volunteers on a monthly basis.
2. Items owned by the Mansfield Public Library may be borrowed for extended loan periods if necessary.
3. Homebound residents are also encouraged to make their own selections, clicking on the "catalog" button on the left side of this page.
